If your doctor determines that you qualify, they fill out two separate forms –– a waiver form and a certification form –– and submit them to the state. Once the state processes your application, a representative from the Department of Public Health’s Vital Records contacts you.

A medical marijuana card costs $25. Before receiving your card, you’ll need to submit the payment to the Public Health Office, where you pick it up. 

Each card is valid for two years from the date you receive it. When it expires, you’ll need to reapply.
